I think we have all asked that very same question and the answer is a mix of a few standard issues. The hard part has always been deciding how to move it forward. Without the customers demanding change hosts wont do it, without the hosts support application developers are reluctant to move to PHP 5 only versions. One of the issue's cited was lack of popular opensource projects supporting PHP 5, maybe we should all encourage our favorite packages to sign up to http://gophp5.org/ as this seems like an interesting idea on how to speed up the migration.
